Service Medical Director, Specialty Surgery, Monash Health
Adjunct Clinical Professor, Monash University

Professor Leon Lai is a Melbourne neurosurgeon specialising in complex brain, cerebrovascular and skull base surgery. He has performed more than 2,400 neurosurgical procedures, including over 1,800 complex cranial operations, with particular expertise in brain aneurysms, arteriovenous malformations (AVMs), dural arteriovenous fistulas (dAVFs), brain cavernomas, brain and skull base tumours, acoustic neuromas, meningiomas and trigeminal neuralgia.

His dedicated referral practice focuses on complex cranial neurosurgery. Patients are referred from across Melbourne, regional Victoria, throughout Australia and overseas for primary treatment and second opinions, including uncommon, recurrent and technically challenging conditions.

Cerebrovascular & Skull Base Expertise

As Head of Cerebrovascular & Skull Base Surgery at Monash Health, Professor Lai has extensive experience in the assessment and surgical management of complex cerebrovascular and skull base disorders.

His approach begins with an accurate diagnosis, understanding the natural history of the condition and determining whether intervention is truly necessary. Many patients do not require surgery. Where treatment offers meaningful benefit, options may include microsurgery, endovascular therapy, stereotactic radiosurgery or multidisciplinary care.

For conditions where more than one treatment strategy may be appropriate, Professor Lai works closely with interventional neuroradiologists, neurologists, neuro-oncologists, radiation oncologists and other specialists to develop an individualised treatment plan. The objective is to select the treatment that offers the greatest long-term benefit while avoiding unnecessary intervention.

Surgical Experience & Approach

When surgery is recommended, meticulous microsurgical technique, careful operative planning and preservation of neurological function are central to Professor Lai’s practice.

Where appropriate, less invasive approaches are used to minimise unnecessary tissue disruption and brain retraction without compromising the objectives of surgery. Careful attention is also given to wound closure, cosmetic outcome and recovery. Successful surgery is measured not simply by the radiological result, but by neurological preservation, recovery and long-term quality of life.

Academic & Professional Leadership

Professor Lai combines a busy clinical practice with senior roles in surgical leadership, research and education.

He is Service Medical Director – Specialty Surgery at Monash Health and Adjunct Clinical Professor at Monash University. He serves on the Court of Examiners of the Royal Australasian College of Surgeons and is Associate Editor of the Journal of Clinical Neuroscience.

He has authored more than 120 peer-reviewed publications spanning cerebrovascular surgery, skull base surgery, brain tumours and neurosurgical outcomes, and remains actively involved in clinical research, surgical education and the training of future surgeons.

Clinical Philosophy

Professor Lai believes that good neurosurgery requires both technical expertise and sound clinical judgement. The important question is not simply whether an operation can be performed, but whether it should be performed and which treatment offers the greatest long-term benefit.

Patients receive clear explanations of their diagnosis, its natural history and the available treatment options, allowing them to make informed decisions about their care. Where observation is appropriate, surgery is not recommended. Where intervention is necessary, treatment is tailored to achieve the best possible outcome while minimising unnecessary risk.
